The church kept the West Syriac liturgy, the threefold ministry of bishop, priest and deacon, the church year and the fasts, and changed the theology inside them. It calls itself Reformed Oriental and is in full communion with the Anglican churches and with the Church of South India, not with any Oriental Orthodox church.

The Mar Thoma Church is a genuine hybrid and resists every available label. Its liturgy is the Anaphora of Saint James, its bishops stand in the West Syriac succession, its vestments and chant are Eastern, and its doctrine of justification, its rejection of the invocation of saints and its treatment of scripture are Reformation positions. It is in full communion with the Anglican Communion and with the Church of South India and the Church of North India, and its metropolitan sits in Anglican primates' gatherings. It is not in communion with the Malankara Orthodox, Jacobite, or any Oriental Orthodox church, though relations are cordial and intermarriage is common.
